Evidence on the Use of Retinoids in the Treatment of Acne

Abstract

Introduction: Acne is a chronic, multifactorial inflammatory disease of the pilosebaceous unit. Its development is associated with various factors, such as excessive sebum production, follicular hyperkeratosis, bacterial colonization, and an innate or adaptive inflammatory response. It is estimated that around 650 million people worldwide suffer from some degree of acne. In Chile, approximately 70% of adolescents have acne. Despite the availability of first-line therapies, knowledge gaps persist, particularly regarding their effectiveness. Objective: A search for evidence on the use of retinoids in acne treatment was conducted. Methodology: A systematic review of articles from Web of Science (2015 to 2025), in both Spanish and English, using keywords and Boolean markers. Results: Retinoids have shown high efficacy in reducing acne lesions, while also improving post-inflammatory pigmentation and overall skin quality. Combined and encapsulated formulations enhance tolerability and adherence to treatment. Their use in the maintenance phase significantly reduces relapses following intensive therapies such as oral isotretinoin. Discussion: Topical and oral retinoids have established themselves as key treatments for acne due to their effectiveness, tolerability, and aesthetic benefits. However, methodological limitations persist in the available evidence. Conclusion: Topical retinoids are effective and well-tolerated in the treatment of mild to moderate acne, offering additional benefits in pigmentation and barrier function. More standardized studies are needed to confirm their efficacy in new formulations and in more severe cases.
