Prostatic Infiltration by Adult T-Cell Leukemia/Lymphoma (HTLV-1+): A Case Report

Unlike the frequent adenocarcinomas derived from the ductal epithelium, prostatic involvement by lymphoma or leukemia represents an unusual variant of urologic neoplasia. These conditions may be detected following an abnormal digital rectal examination or an increase in serum prostate-specific antigen (PSA). The present case is that of a 49-year-old man, positive for human T-cell lymphotropic virus type 1 (HTLV-1), with lower urinary tract symptoms (LUTS) and persistently elevated PSA, in whom T-cell lymphoma with prostatic involvement was confirmed. The diagnosis was supported by an immunohistochemical profile strongly positive for CD4, CD5, CD25, and BCL2, and negative for pancytokeratin, CD20, and other T-cell/lymphoid markers (CD7, CD8, CD10, CD30, ALK1, and EBER). Although exceptional (<0.1% of prostatic neoplasms), lymphomas and leukemias involving the prostate should be included in the differential diagnosis when the clinical or imaging findings are atypical for adenocarcinoma.
